Health care reform: What prevention really means

With the President's address to Congress on health care reform yesterday, the grass roots are in a highly energized state. I have received at least 4 separate e-mails yesterday from 4 separate organizations calling on me to advocate for an emphasis on prevention. Call Congress, write the President, let your feelings be known!Well, my feelings are in fact these: We need a bracing dose of humility. The foundations of “health” are not built in hospitals or clinics. They are built in kitchens and family rooms; in schools and neighborhoods; and in supermarkets and backyards. Here's what I think:1) I believe basic access to needed care is a human right, so universal coverage -- by one means or another -- is a must. 2) I believe we can do far better at standardizing quality, and by so doing, saving both lives and dollars.3) I believe that how people live their lives each day—whether or not they smoke, what they eat, whether or not they are active, and so on—is the master lever of medical destiny. To look for the true power of disease prevention and health promotion in so-called “health care reform” is to look well beyond its horizon. Health care matters most when things go wrong, but can do relatively little at the points of true origin to ensure things go right. My worry is that in expecting too much of health care reform, we may simply fail to achieve any aspect of it. We need health care reform to ensure that what clinicians can do, they do well—and for everyone. But we must acknowledge that clinicians alone cannot build the foundations of health. For that, we need reforms that reverberate through our culture and society, to the places we actually live out the lives we are trying to protect.

What to Do When Life Becomes too Exhausting?

by SINGLE EDITION, on Tue Jun 30, 2009 7:37am PDT

When my phone rang the other week I was excited to hear the voice on the other end. It was Diane, my first roommate in Montreal who had disappeared from my life when she decided to go to culinary school several years ago. Now a professional chef, this former take-out junkie was painting pastries while I was still focused on helping millions of Americans cut out their carbs during my extended stint in the diet industry.
It only took minutes before she blurted out the words "I'm exhausted," which for me conjured images of a primer, plumper Demi Moore from that scene in St. Elmo's Fire where she said something along the lines of "I never thought I would be this tired at 24." But at 37, it was not a make-believe Hollywood coke habit that Diane was tired of supporting, it was the demands of life faced on her own.

There's no denying that the constant pressures of single living can slowly chip away at the soul. Almost everyone I know, male or female, has reached that tipping point at least once in their lives. But you know the saying "too much of anything is not always a good thing?" The same principle holds true when it comes to your life. That's because being over or poorly committed will often trigger anxiety as well as feelings of detachment and disappointment. When this happens, you need to examine if you are being properly nourished by yourself and those around you.
A good place to start is by checking your calendar. If it's always booked solid with superfluous appointments or plans, then keeping busy may be a coping mechanism you use to avoid being alone or disappointing those around you. The problem is that being "occupied" does not necessarily yield the return on investment you need to feel satisfied. To the contrary, it can leave you emotionally and psychologically empty.
Amidst all the obligations, it's easy to lose touch with yourself. The key to staying centered is finding time for commitments that matter and mean something to YOU. When the world gets too overwhelming, something as simple as touching a wall or taking a shower can help restore a sense of connectedness. That's why it is important to find outlets for creative expression, physical activity and contemplation.

Yawning? 6 Surprising Reasons Why You're Exhausted

Sure, getting enough sleep is the biggest way to beat the 4:00 yawning session (I swear, there is something about this time--weirdly, I could fall asleep at 4 p.m. everyday!), but did you know that several other surprising things could be making you sleepy? Here are some unlikely yawn-inducers...
WebMD recently ran a piece about the most common reasons for feeling tired. Here are some:
1. A hidden UTI: In some cases, fatigue may be the only sign of a urinary tract infection, reports WebMD. If you suspect something's up (and you have pain and burning when you pee) talk to your doc. A urine test can quickly confirm a UTI.
2. Your diet: If you're not eating enough, you will be tired. Calories = energy, my friends. (I feel like I sounded just like John McCain when I wrote that.)
3. Food allergies you didn't know you had: Some doctors believe that exhaustion could be a sign of hidden food allergies.
4. Caffeine overload: You probably reach for more coffee when you're tired, but experts say that too much caffeine can sometimes backfire, causing you to feel more fatigued. Maybe skip the triple-venti today.
5. Undetected thyroid problems: Fatigue is one symptom of a thyroid condition called hypothyroidism. Fortunately, this is such a treatable thing (I have it, and just pop a pill each day--no biggie). Most health experts concur that every woman should have her thyroid levels tested every few years (just a simple blood test) to rule out any such conditions (they're common in women).
P.S. I'd add a sixth: your snoring man. My husband snores, and it makes it so hard for me to get to sleep most nights. I told you about a new product called Brez that he was going to try. Well, he tried it! The verdict: While it didn't stop his snoring completely, it has helped. Somehow the snoring has gotten softer and less intense. Hey, I'll settle for any improvement!
Are you sleepy today? What do you blame your fatigue on? The baby? Your job? Stress?
